NEW PATIENTS
Thank you for choosing the Fall Injury Prevention and Rehabilitation Center. We look forward to serving you with exceptional care. For your convenience, you may complete new patient forms in advance. Please select and print the new patient packet below. Fill all forms out completely and bring them with you to your appointment. If you are not able to complete the forms before your visit, please arrive at least 30 minutes early for completion of patient paperwork in the office before you see the doctor.
Please bring the following items with you to your appointment:
- Current Medications - Please bring all medications you are currently taking to your appointment
- Personal Identification Needed - Be sure to have your valid driver license
- Insurance card(s) – Please bring your current insurance card
